Child Information Card:  

Please complete the child information card and send in on the first day of school 

Child Information, Emergency Contacts, and Approved Pick-ups (skip if you did this for camp)

1.     Sign into Brightwheel through the website and not the app

2.     Click Profile

3.     Click Edit in the gray information box labeled Personal Information

4.     Complete all requested information

5.     Click Save

6.     Check and Update your Address in the next gray box

7.     Click edits in the contact information

8.     Add in both parents and complete all requested information (these are also approved pick-ups)

9.     Click +Add a contact and add 2 EMERGENCY CONTACTS, complete all requested information (these are also approved pick-ups)

10.  Click +Add a contact and add any other approved pick-up people, complete all requested information

Health Appraisal and Immunization record (skip if you did this for camp)

1.     Please take the attached Health Appraisal document to your child’s doctor for completion

2.     Send in with your child on their first day of school

 Sunblock, Bug Spray, and Diaper Cream

Please download the attached MAR and print and complete one per item listed above if you would like our teachers to reapply sunscreen, bug spray, or diaper cream while your child is here.  You must provide the sunscreen, bug spray, and diaper cream labeled with your child’s name.  We cannot reapply without this form and without the product.
